2. Schedule an appointment

• Text us in Spruce. Dr. Krebs monitors her Spruce Inbox very closely. This is the fastest way to reach her for non-urgent medical concerns.
• Call Dr. Krebs’ Current Patient Line directly at 808-374-5620. Please save this number to your phone to enable you to reach Dr. Krebs as needed. This line is only available to current members.

3. Medical History Form:
Once you schedule your first appointment, vou will receive an email requesting vou complete our medical history
form electronically prior to your visit.

4. Past Medical Records:
If you would like us to obtain your past medical records, please submit the Record Request Form to your past provider.

5. Access your health information:
After our first visit, you will receive a link to access the Elation patient portal.
